Founded in 2004, HealthLinkRx is a healthcare consulting firm dedicated to helping a wide array of healthcare organizations in the areas of recruiting, training and management services.  We partner with physicians, payers, insurance companies, hospices and home health agencies to provide in-home nurse practitioner visits for Medicare and Medicare Advantage members and typically work with insurance companies and/or large self funded employers to assist them in proactively managing the medical risk and quality of life for their members.

Our goal is to identify and treat patients with one or more chronic conditions and proactively manage the medical risk and quality of life for patients. The way we are able to achieve this goal is through our Nurse Practitioners. Our Nurse Practitioners make in-home visits to patients to conduct assessments to gather health related information, provide education and identify potential health issues early.
